Epitop Tablets contain Topiramate 25 mg, 50 mg, or 100 mg. Topiramate is a broad-spectrum antiepileptic agent with multiple mechanisms of action that help prevent the initiation and spread of seizures.

Epitop Tablets provide effective management of both partial-onset and generalized tonic-clonic seizures as monotherapy or adjunctive therapy. Topiramate exerts anticonvulsant activity through multiple mechanisms, including sodium channel blockade, enhancement of GABA-mediated inhibition, antagonism of glutamate receptors, and inhibition of carbonic anhydrase. This multimodal mechanism helps reduce seizure frequency while offering broad-spectrum antiepileptic efficacy.

Epitop Tablets are indicated for the treatment of partial-onset seizures and generalized tonic-clonic seizures in adults and appropriate pediatric patients.

Topiramate exerts its antiepileptic activity through multiple complementary mechanisms. It blocks voltage-dependent sodium channels, enhances the activity of the inhibitory neurotransmitter gamma-aminobutyric acid (GABA) at selected GABA-A receptor subtypes, antagonizes the AMPA/kainate subtype of glutamate receptors, and inhibits carbonic anhydrase isozymes II and IV. These combined actions stabilize neuronal membranes, suppress excessive neuronal firing, and reduce seizure propagation.

In adults and pediatric patients 10 years of age and older, the recommended starting dose is 50 mg/day administered in two divided doses. The dose should be increased weekly by 50 mg/day during the first four weeks and by 100 mg/day during weeks five and six to a recommended maintenance dose of 400 mg/day in two divided doses. In children 2 to less than 10 years of age, therapy should begin with 25 mg once daily at night during the first week, followed by gradual dose titration over 5 to 7 weeks according to body weight. For adjunctive therapy in adults, the recommended total daily dose is 200–400 mg/day in two divided doses for partial-onset seizures and 400 mg/day in two divided doses for primary generalized tonic-clonic seizures. Doses above 400 mg/day have not demonstrated additional clinical benefit.

Epitop Tablets are contraindicated in patients with known hypersensitivity to Topiramate or any component of the formulation.
