i-Phytoral Capsules contain Itraconazole, a broad-spectrum triazole antifungal agent effective against a wide range of systemic and superficial fungal infections. It inhibits fungal ergosterol synthesis, providing potent antifungal activity against Aspergillus, Candida, Cryptococcus, and other pathogenic fungi. The formulation is suitable for the treatment of systemic mycoses, onychomycosis, and oral candidiasis.
Indications
i-Phytoral Capsules are indicated for the treatment of systemic aspergillosis, candidiasis, cryptococcosis, sporotrichosis, paracoccidioidomycosis, blastomycosis, onychomycosis, oral candidiasis, and other rare systemic or tropical mycoses.
Pharmacodynamics
Itraconazole is a triazole antifungal agent that inhibits the fungal cytochrome P450-dependent 14-α-demethylase enzyme, preventing the conversion of lanosterol to ergosterol, an essential component of the fungal cell membrane. Depletion of ergosterol disrupts membrane integrity, inhibits fungal growth, and leads to fungal cell death.
Dosage and administration
For blastomycosis and histoplasmosis, the recommended dose is 200 mg once daily, which may be increased to a maximum of 400 mg daily. For aspergillosis, a daily dose of 200–400 mg is recommended. For toenail onychomycosis, the recommended dose is 200 mg once daily for 12 consecutive weeks. For fingernail onychomycosis, the recommended regimen consists of 200 mg twice daily for one week, followed by a three-week drug-free interval, repeated for two treatment pulses. For oral candidiasis, the recommended dose is 100 mg once daily for 15 days, or as directed by the physician.
Contraindications
i-Phytoral Capsules are contraindicated in patients with known hypersensitivity to Itraconazole or any component of the formulation. They should not be used for the treatment of onychomycosis in patients with evidence or a history of congestive heart failure (CHF). Concomitant administration with cisapride, pimozide, quinidine, dofetilide, or levacetylmethadol (levomethadyl) is contraindicated.
