Empagliflozin + Linagliptin + Metformin SR Tablets combine three complementary antihyperglycemic agents in a single once-daily formulation for comprehensive management of type 2 diabetes mellitus. Empagliflozin enhances urinary glucose excretion by inhibiting SGLT2, Linagliptin prolongs incretin activity by inhibiting DPP-4 to improve glucose-dependent insulin secretion, and Metformin reduces hepatic glucose production while improving insulin sensitivity. This triple therapy helps achieve sustained glycemic control while simplifying treatment and improving patient adherence.

Empagliflozin + Linagliptin + Metformin SR Tablets are indicated as an adjunct to diet and exercise for improving glycemic control in adults with type 2 diabetes mellitus.

Empagliflozin selectively inhibits sodium-glucose co-transporter-2 (SGLT2), reducing renal glucose reabsorption, lowering the renal threshold for glucose, and increasing urinary glucose excretion. Linagliptin inhibits dipeptidyl peptidase-4 (DPP-4), thereby increasing active concentrations of incretin hormones such as GLP-1 and GIP, resulting in enhanced glucose-dependent insulin secretion and suppression of glucagon release. Metformin is an antihyperglycemic agent that decreases hepatic glucose production, reduces intestinal glucose absorption, and improves peripheral insulin sensitivity, thereby lowering both fasting and postprandial blood glucose levels.

The maximum recommended daily dose is Empagliflozin 25 mg, Linagliptin 5 mg, and Metformin Hydrochloride Extended Release 2000 mg. The tablet should be taken once daily with the morning meal and swallowed whole without crushing, splitting, or chewing. Dosage should be individualized according to the patient's glycemic response and renal function.

This combination is contraindicated in patients with severe renal impairment (eGFR below 30 mL/min/1.73 m²), end-stage renal disease, or those receiving dialysis. It is also contraindicated in patients with metabolic acidosis, including diabetic ketoacidosis, and in individuals with known hypersensitivity to Empagliflozin, Linagliptin, Metformin, or any component of the formulation.
