Empagliflozin Tablets contain a selective sodium-glucose co-transporter-2 (SGLT2) inhibitor that improves glycaemic control by reducing renal glucose reabsorption and increasing urinary glucose excretion. By lowering the renal threshold for glucose, empagliflozin effectively reduces blood glucose levels independent of insulin secretion. It also promotes natriuresis and osmotic diuresis, contributing to cardiovascular and renal benefits in appropriate patients with type 2 diabetes mellitus. The once-daily oral formulation provides convenient dosing and supports long-term diabetes management.
Indications
Empagliflozin Tablets are indicated as an adjunct to diet and exercise to improve glycaemic control in adults with type 2 diabetes mellitus. They may be used as monotherapy or in combination with other glucose-lowering medicinal products, including insulin, when additional glycaemic control is required.
Pharmacodynamics
Empagliflozin is a selective inhibitor of sodium-glucose co-transporter-2 (SGLT2), the primary transporter responsible for renal glucose reabsorption from the glomerular filtrate. By inhibiting SGLT2, empagliflozin reduces renal glucose reabsorption, lowers the renal threshold for glucose, and increases urinary glucose excretion, thereby lowering plasma glucose concentrations. In addition, empagliflozin promotes sodium excretion and increases sodium delivery to the distal renal tubule, which may contribute to reductions in cardiac preload and afterload and modulation of sympathetic nervous system activity.
Dosage and administration
The recommended starting dose is 10 mg once daily, taken orally with or without food for monotherapy or combination therapy. In patients who tolerate the 10 mg dose, have an eGFR ≥60 mL/min/1.73 m², and require tighter glycaemic control, the dose may be increased to 25 mg once daily. The maximum recommended daily dose is 25 mg. When used with insulin or sulfonylureas, dose reduction of these agents may be considered to reduce the risk of hypoglycaemia.
Contraindications
Empagliflozin is contraindicated in patients with known hypersensitivity to empagliflozin or any component of the formulation and in patients receiving dialysis.
